As per Fabrizio Romano, Chelsea and West Ham have reached an agreement over the transfer of Emerson.
Emerson will sign a five-year contract with the Hammers, keeping him at the club until 2027.
In terms of a transfer fee, Emerson will set West Ham back £13m plus £2m in add-ons.
It is added that the first part of Emerson’s medical will be completed on Monday.

Emerson joined Chelsea from Serie A side AS Roma back in January of 2018 for £18m.
However, during his spell in England, he really struggled to hold down a first-team spot under a number of managers.
Emerson was a regular feature for the French outfit, scoring one and assisting two in 36 outings.
He has since returned to Chelsea and is firmly down Thomas Tuchel’s pecking order.
Chelsea signed Marc Cucurella from Brighton this summer, meaning the Blues currently have four left-backs on their books.
Marcos Alonso was expected to depart for FC Barcelona, but that deal has now stalled.
And even if the Spaniard left, Emerson was still unlikely to see any minutes ahead of Cucurella and Ben Chilwell.
